COVID-19 BOOKING CONDITIONS   Level 2

Please do not attend the clinic  if you can answer ‘Yes’ to ANY of the following screening questions:

1. Have you tested positive for COVID-19

2. Have you had contact with anyone with confirmed COVID-19, or someone who has been in direct contact     with a confirmed case, in the last 14 days?

3. Have you had any of these symptoms in the last 14 days?
4. Are you currently experiencing fever over 38°C, difficulty breathing, cough or loss of sense of smell or taste?

 * Parking Area for customers *

Customers can park on the driveway by the garage or close by the driveway. To avoid an unwanted fine, please find area where there is no yellow lane. 

 

 Cancellation Policy

Your appointments and well-being are very important to us. We understand that sometimes, unexpected delays can occur, making schedule adjustments. If you need to cancel your appointment, we respectfully request at least 24-hour

With respect and consideration to other patients and the practitioners’ time, if a patient does not show up for a scheduled appointment and no reasonable explanations provided, the patient will be charged the full price of the scheduled visit.
